Invisible at Work: How American Occupational Health Systems Are Overlooking Tuberculosis in High-Risk Industries

Every morning, millions of American workers clock in without any awareness that a colleague beside them may be carrying an undetected tuberculosis infection. For most industries, that risk is abstract. For healthcare workers, correctional officers, food service employees, and agricultural laborers, it is a documented and statistically significant reality — one that occupational health programs across the country are consistently failing to address.

The United States has long maintained formal TB screening guidelines through the Centers for Disease Control and Prevention (CDC) and the Occupational Safety and Health Administration (OSHA). Yet the translation of those guidelines into consistent, enforceable workplace practice remains alarmingly incomplete. The result is a structural blind spot: high-risk workers cycle through annual wellness checks, biometric screenings, and corporate health portals without ever receiving a tuberculosis-specific assessment appropriate to their exposure profile.

A Fragmented System Built on General Principles

American occupational health as a discipline evolved largely around injury prevention, chemical exposure, and chronic disease risk — not infectious disease surveillance. The modern corporate wellness program, now standard in mid-to-large employers, tends to prioritize metrics such as blood pressure, cholesterol, body mass index, and smoking cessation. TB screening, when it appears at all, is frequently reduced to a single self-reported symptom questionnaire rather than a tuberculin skin test or an interferon-gamma release assay (IGRA), both of which are considered gold-standard diagnostics.

"The problem is that general wellness platforms were never designed with tuberculosis in mind," explains one occupational health physician with experience across hospital systems in the Southeast. "They are optimized for chronic disease management and liability reduction. TB requires a different clinical mindset — one oriented around exposure risk, population epidemiology, and infectious disease protocols."

This philosophical mismatch between general wellness objectives and TB-specific clinical requirements creates a gap that infected workers can fall through entirely, sometimes for years.

High-Risk Industries, Low Screening Compliance

Three industries stand out in the public health literature as particularly vulnerable to TB transmission: healthcare, corrections, and food service. Each presents a distinct occupational exposure profile, and each is failing its workforce in different ways.

Healthcare settings, despite being subject to the most rigorous federal guidance, have seen a notable erosion in screening discipline following the COVID-19 pandemic. Many hospital systems suspended routine TB screening during 2020 and 2021 and have not fully reinstated structured programs. A 2023 survey of infection control practitioners found that nearly one in five acute care hospitals reported inconsistent baseline TB testing for new hires in clinical roles.

Correctional facilities represent perhaps the most acute occupational TB risk in the country. Incarcerated populations experience TB rates significantly higher than the general public, and correctional officers work in close, poorly ventilated proximity to those populations daily. Yet correctional officer TB screening programs vary widely by state and institution, with some facilities relying on self-reported symptom monitoring rather than annual diagnostic testing. Officers who develop active TB may work symptomatic for weeks before any clinical assessment occurs.

Food service and agricultural workers, particularly those employed in meatpacking plants, poultry processing facilities, and large-scale restaurant operations, face a different set of barriers. This workforce skews heavily toward immigrant and migrant communities — populations that carry a disproportionate TB burden due to country-of-origin prevalence rates and limited prior access to preventive care. Employer-sponsored health benefits in these sectors are inconsistent, and TB screening is rarely integrated into onboarding or annual health protocols.

The Policy Vacuum Enabling Ongoing Exposure

At the federal level, no OSHA standard specifically mandates TB screening for workers in high-risk settings outside of healthcare. The agency's General Duty Clause, which requires employers to provide a workplace free from recognized hazards, has theoretically been applied to TB in isolated enforcement actions — but it has never served as the foundation for systematic, sector-wide screening requirements.

The CDC's Guidelines for Preventing the Transmission of Mycobacterium tuberculosis in Health-Care Settings, last comprehensively updated in 2005, remain the most detailed federal framework available. However, these guidelines carry no enforcement mechanism and apply only to healthcare environments. For corrections, food service, and other high-risk industries, no equivalent federal standard exists.

State-level mandates fill some of this void, but inconsistently. California, New York, and Texas — states with among the highest TB case counts in the nation — each maintain different requirements for occupational TB screening, creating a regulatory patchwork that confuses employers and leaves workers in lower-compliance states without meaningful protection.

What Effective Occupational TB Surveillance Looks Like

The disconnect between policy intent and operational reality is not inevitable. Several health systems and correctional departments have implemented TB surveillance programs that serve as instructive models.

The Los Angeles County Department of Public Health, working in partnership with correctional health services, established a systematic TB screening protocol for correctional staff that includes baseline IGRA testing upon hire, annual follow-up for those in direct inmate contact, and a clear clinical pathway for workers who convert from negative to positive. The program's success rests not on individual compliance but on institutional infrastructure — dedicated occupational health staff, electronic tracking systems, and defined escalation procedures.

Similarly, several large academic medical centers have moved beyond annual symptom questionnaires to implement risk-stratified screening protocols that assign testing frequency based on documented exposure levels. Workers in high-exposure units receive more frequent assessment than those in administrative roles — a clinically rational approach that maximizes sensitivity while managing resource costs.

"The data consistently show that when you build TB screening into the workflow rather than asking workers to opt in, detection rates improve substantially," notes a public health researcher who has studied occupational TB programs across multiple health systems. "It is an infrastructure problem more than a behavioral one."

Moving from Awareness to Accountability

Addressing the occupational TB screening gap requires action on multiple fronts simultaneously. Federal agencies, particularly OSHA and the CDC, must collaborate on updated, enforceable guidance that extends beyond healthcare to cover all industries where documented TB exposure risk exists. Employer accountability mechanisms — whether through OSHA enforcement, accreditation standards, or public health licensing requirements — are essential to move the needle beyond voluntary compliance.

Occupational health professionals, for their part, need continuing education resources that integrate TB-specific protocols into general wellness program design. Industry associations in corrections, food service, and agriculture have a role to play in developing sector-specific screening frameworks that are practical for employers and protective of workers.

For the StopTB Initiative and partner organizations, the workplace represents an underutilized point of intervention. Millions of high-risk individuals pass through employer-sponsored health programs each year — a population access point that, properly leveraged, could dramatically improve early detection rates and interrupt transmission chains before they extend beyond the worksite.

The cost of inaction is not theoretical. Every undetected case in a healthcare worker, a corrections officer, or a food service employee represents not only a delayed diagnosis for that individual but a potential exposure event for dozens of colleagues, patients, or customers. Closing the occupational screening gap is not an administrative nicety. It is a public health imperative.
