Tuberculosis is often described as a disease of adults — a condition conjured by images of crowded shelters, correctional facilities, and immigration detention centers. Yet children contract TB at rates that American public health infrastructure has been slow to acknowledge, and the consequences of missing a diagnosis in a young patient can ripple outward for months before anyone connects the dots. When a child goes undiagnosed, that child continues to live, sleep, and attend school alongside others, all while a preventable infection progresses silently.

In the United States, pediatric TB cases account for a meaningful proportion of the national burden, and a significant share of those cases involve children under the age of five — the group most vulnerable to severe or disseminated disease. Yet the systems designed to catch TB in young patients remain fragmented, inconsistently applied, and poorly resourced. Understanding why children fall through the cracks requires examining the biology of pediatric TB, the limits of current clinical practice, and the structural gaps in American pediatric care.

Why TB Looks Different in Children

The hallmark symptoms of tuberculosis in adults — a persistent productive cough, night sweats, unexplained weight loss — are far less reliable indicators in children. Young patients, particularly those under ten, frequently present with vague constitutional symptoms: mild fever, fatigue, reduced appetite, or a cough that a busy clinician might reasonably attribute to a respiratory virus or allergic rhinitis. In infants and toddlers, TB can progress rapidly to miliary disease or tuberculous meningitis before a pulmonary diagnosis is even considered.

Compounding this diagnostic ambiguity is the fact that children with TB are less likely than adults to be sputum smear-positive, making laboratory confirmation considerably harder to obtain. Collecting adequate sputum samples from young children is technically challenging, and gastric aspirate cultures — a more reliable method in this population — require hospitalization and procedural expertise that many outpatient pediatric practices cannot offer. The result is a diagnostic pathway that depends heavily on clinical suspicion, a commodity that is unevenly distributed across the American healthcare system.

The Clinician Familiarity Problem

Many American pediatricians complete their training without ever diagnosing a case of active tuberculosis. In low-prevalence states, a general practitioner might practice for decades without encountering a confirmed pediatric TB case. This unfamiliarity has real consequences. Studies have documented that the interval between symptom onset and diagnosis in children with TB is frequently measured in weeks or months, not days. During that window, disease advances, treatment is delayed, and household contacts — including other children, grandparents, and immunocompromised family members — remain at risk.

The challenge is not a lack of capable clinicians. It is, rather, a predictable consequence of low background prevalence combined with high diagnostic complexity. When a condition is rare enough that a practitioner never develops pattern recognition for it, clinical intuition fails to compensate for the absence of systematic screening. This is precisely why evidence-based protocols, rather than individual clinical judgment alone, must anchor pediatric TB detection efforts.

Where Screening Falls Short

The American Academy of Pediatrics recommends risk-based TB screening during routine well-child visits, using a structured questionnaire to identify children with exposure histories, immigration backgrounds, or living situations that elevate their probability of infection. In theory, this approach targets resources toward the children who need them most. In practice, implementation is inconsistent.

Clinics serving high-risk populations — including federally qualified health centers in urban cores, migrant health programs, and practices near the southern border — often lack the staffing, time, or reimbursement structures to conduct thorough risk assessments at every visit. Electronic health record systems vary widely in their capacity to prompt providers toward TB screening questions. And in communities where TB carries significant social stigma, families may underreport exposures even when directly asked.

Latent tuberculosis infection, which can affect children who have been exposed to an infectious adult without developing active disease, is particularly prone to being missed. A child with latent TB infection harbors a future risk of progression to active disease — a risk that is highest within the first two years after infection and remains elevated throughout childhood. Treating latent infection in children is both safe and highly effective, but treatment cannot begin if the infection is never identified.

The Household Connection

One of the most reliable pathways to pediatric TB diagnosis is contact investigation following an adult index case. When a parent, grandparent, or household member is diagnosed with active pulmonary TB, public health protocols call for systematic evaluation of all close contacts, including children. This process, when executed promptly and thoroughly, can identify infected children before symptoms appear and initiate preventive therapy early.

However, contact investigation capacity varies dramatically across US health departments. Jurisdictions with robust TB programs and adequate staffing conduct thorough household investigations within days of a new diagnosis. Others, particularly in states where TB caseloads are low and public health infrastructure has been reduced over years of budget constraints, may take weeks to complete the same process — or may struggle to locate and engage all relevant contacts at all. Children in these gaps are the ones most likely to progress from latent infection to active disease without intervention.

Evidence-Based Strategies for Pediatric Practice

Pediatricians do not need to wait for systemic reform to improve TB detection in their own practices. Several evidence-based strategies can meaningfully reduce diagnostic delay at the clinical level.

First, structured risk-factor screening at every well-child visit — using validated questionnaires that account for birth country, household contacts, immigration history, and congregate living situations — increases the probability of identifying children who warrant tuberculin skin testing or interferon-gamma release assay. Second, when a child presents with a prolonged cough, unexplained fever lasting more than two weeks, or failure to thrive without a clear alternative explanation, TB should be included in the differential diagnosis regardless of perceived prevalence in the local community. Third, pediatric practices serving high-risk populations should establish clear referral pathways to TB specialists or local public health departments, reducing the time between clinical suspicion and confirmatory workup.

Training also matters. Continuing medical education focused on pediatric TB recognition, available through organizations including the American Academy of Pediatrics and the Centers for Disease Control and Prevention, can help practitioners in low-prevalence settings maintain clinical readiness for a disease they may rarely encounter.

A Preventable Failure With Preventable Consequences

The gap in pediatric TB detection is not a mystery. Its causes are well understood: diagnostic complexity, clinician unfamiliarity, inconsistent screening, and underfunded contact investigation. What remains insufficient is the collective will to address these causes with the urgency they demand.

Children diagnosed late with active TB face longer treatment courses, greater risk of severe complications, and the burden of a diagnosis that could have been prevented or caught far earlier. Their families face months of disruption, fear, and uncertainty. And communities absorb the downstream costs of transmission that might have been interrupted at the source.

America's pediatric healthcare system has the tools, the knowledge, and the infrastructure to do better. Closing the TB testing gap in children is not a matter of waiting for new technology or new drugs. It is a matter of applying what is already known, consistently and equitably, to the youngest and most vulnerable patients in the country.
